We’re in the early stages of developing a digital platform designed to empower social prescribers, care coordinators, and other non-clinical roles. TPL is looking to offer a secure, real-time system to streamline communication, support coordination, and centre care around the individual. We’re keen to open up the conversation about what’s needed and explore how this digital tool could evolve with input from partners like you.
Overview
The aim of this session is to explore how digital tools can better support personalised care roles such as care coordinators and social prescribers. Key themes include improving communication, enabling real-time person-centred records, and empowering patients and staff alike. This is important because non-clinical roles are growing rapidly, yet they often lack the digital infrastructure needed to deliver truly joined-up, proactive support.
